I helped a lady make one for her family. She used this site:

It came out nice, and they were easy to work with. Can't remember how pricey it was, because I didn't pay for it—I just did the work. :)

I don't know anything about cookbooks, but my partner looked at a lot of POD services for his self-published books on editing and chose Lulu.com.

We used Lulu a couple times for printing. Husband was in charge of that, but we've been happy.

I used Lulu.com for a family project, and found the quality quite nice, especially if you're comfortable enough with graphic design to manage your own project.
