OK. I have various ways I picture John's relationship history, but one picture that I rather like is the one where he's actually telling the truth as he understands it when he keeps saying, "I'm not gay."
Now, with women I see John as genuinely a terrific lover -- experienced and open-minded and quite good at sexual communication, the sort of person who makes his partners into better lovers. I'm also going to say that he's probably tried most of the things that a man can do with a woman (or two), so it's not that something like anal is going to throw him.
But going to bed with Sherlock (as one must, if one is John Watson) is in some ways going to pop him back to adolescence. He doesn't know how to interpret arousal signs in a male body. He doesn't know which acts another man might find politically or romantically loaded. He's got years of experience handling the female sexual response arc (no reason to delay making her come, because he knows how to get her there again even if she doesn't), but he's not accustomed to having two refractory periods in one bed.
I think he'd also have a straight guy's hangup about sex continuing even when not all the relevant cocks are erect.
